Abstract
The primary cardiac tumor is very rare. Among the most frequent tumors that spread to the heart, mesothelioma is mentioned in the first place 48%, melanoma 27.8% and adenocarcinoma of the lung 21% mainly through the lymphatic route and to a lesser extent, hematogenous1 The presentation depends on the location, generally asymptomatic , but may manifest as dyspnea, cough, chest pain, or cardiac tamponade. Male patient, 71 years old from the capital area of the country. Carrier of heart failure in regular treatment. He comes for 15 days of respiratory distress at moderate efforts, progressively reaching rest. Due to veiling of the left costophrenic sinus on admission chest X-ray, in addition to bilateral hilar thickening, an ultrasound-guided diagnostic thoracentesis was performed (left effusion 400-600cc; right 200-400cc). A chest CT scan shows a mass in the apico-posterior segment of the left upper lobe. In the left lower lobe, mass image with diminished lumen of the segmental bronchi. In the mediastinal window, a thickening of the cardiac contour is observed. In lung biopsy by TRUCUT, poorly differentiated infiltrating squamous carcinoma of large cells returned. He was hospitalized for 22 days. He receives Ampicillin-IBL + Levofloxacin D12 and deworming with nitaxozanide. In the present case, the patient presented with the symptoms described in the literature and cardiac tamponade was confirmed on chest tomography.
