Trauma To The Extrahepatic Bile Ducts And Gallbladder In Patients Who Attended The Emergency Room Of The Regional Hospital Of Ciudad Del Este - Paraguay

Abstract

Injuries to the extrahepatic bile ducts and gallbladder are rare due to their anatomical location. However, these data may be underestimated because initially it is asymptomatic, the diagnosis is clinical suspicion depending on the type and area of trauma, the recommended studies are abdominal spiral tomography and endoscopic retrograde cholangiopancreatography (ERCP), especially in duct lesions. bile The objective was to determine the mechanism of trauma, the site of injury to the extrahepatic bile ducts and gallbladder, associated injuries, evolution, treatment and complications present in patients who attended the emergency department of the Regional Hospital in Ciudad del East - Paraguay from 2010 to 2022. It was a quantitative, retrospective observational analytical study. In 12 years, 7 patients, 6 males, average age 26 years, penetrating trauma in 6. In Laparotomy, the gallbladder was affected in 6 and an extrahepatic bile duct lesion at the level of the right branch at its confluence. The liver, duodenum, stomach, pancreas, right kidney, colon as associated lesions. Managed in the hospital ward and ICU and the complications are leaks, colon necrosis, gastric suture dehiscence, subphrenic collection and septic shock. The trauma of the extrahepatic bile ducts and the gallbladder are rare injuries, the most frequent trauma mechanism is penetrating trauma, perforation of the gallbladder and injury to the right liver, the affectations present, with injuries associated with the liver, duodenum, stomach, pancreas, right kidney and colon. Managed in the hospital ward and in the ICU. Complications are: bile leak, suture dehiscence, colonic necrosis, subphrenic abscess, septic shock.
